How to Claim Bass Pro Shops Outdoor Adventure Second Chance Prizes

In order for the Florida Lottery to complete the processing for your prize, the following items must be received from you:

1) Original, valid, non-winning ticket bearing the ticket number selected in the drawing.
2) Completed and signed Winner Claim Form
3) Copy of acceptable identification that is current or was issued within the past five years and bears a serial number or other identifying numbers. Acceptable forms of identification include:

  • An identification card or driver’s license issued by a public agency authorized to issue driver’s licenses in Florida, a state other than Florida, a territory of the U.S., Canada, or Mexico.
  • A passport issued by the U.S. Department of State.
  • A passport issued by a foreign government.
  • An identification card issued by any branch of the U.S. Armed Forces.
  • An identification card issued by the United States Bureau of Citizenship and Immigration Services.

For Grand Prize winners of the Outdoor Adventure of a Lifetime the following forms are also required:

  1. Signed Release and Authorization Form
  2. Signed Election Form

Adventure Trip winners will be provided a prize certificate with the name of the travel representative from the Lottery’s prize fulfillment company to be contacted for making reservations for the Adventure Trip.

Fulfillment of all prizes will be shipped to the winner’s address within approximately 15 business days after the winning ticket has been received by the Lottery.

Winners must submit all of the above documentation to the Florida Lottery within 180 days of notification by the Lottery of being a winner.

Failure to submit all required documentations will result in forfeiture of the prize.

U.S. Postal Service regulations include various restrictions on mailing Lottery tickets. The risk of mailing tickets remains with the player and will not be assumed by the Lottery.
