How to Claim

If you're a winner of the Jaguars Fan Experience Second Chance drawings, congratulations!

PRIZES VALUED AT $600 OR MORE

The Florida Lottery will attempt to notify each winner of a prize valued at $600 or more (listed below) by U.S. mail or e-mail using the contact information provided in the winner's registration data no later than one week after the winners are drawn. If the Florida Lottery is unable to contact a winner within three weeks of the date of the drawing, the winner will forfeit his or her right to claim the prize and the Florida Lottery will award the prize to an alternate winner.

In order for the Florida Lottery to complete the processing of prizes valued at $600 or more, additional information is required from the prizewinners. If the prize you won is one of the following:

  • Ultimate Jaguars Experience – first prize in the second drawing
  • Game day suite for 16 – first prize in the third drawing
  • Pair of 2011 Lower Level season tickets – first prize in the fifth drawing
  • Pair of 2011 Touchdown Club season tickets – first prize in the sixth drawing

you must submit the following items:

1) Original, valid, non-winning ticket bearing the ticket number selected in the drawing.
2) Completed and signed Winner Claim Form
3) Copy of acceptable identification that is current or was issued within the past five years and bears a serial number or other identifying number. Acceptable forms of identification include:

  • An identification card or driver's license issued by a public agency authorized to issue driver's licenses in Florida, a state other than Florida, a territory of the U.S., Canada, or Mexico.
  • A passport issued by the U.S. Department of State.
  • A passport issued by a foreign government.
  • An identification card issued by any branch of the U.S. Armed Forces.
  • An identification card issued by the United States Bureau of Citizenship and Immigration Services.

4) Signed Release and Authorization Form

Winners of the prizes listed above must submit all of the required documentation to the Florida Lottery within seven (7) days of notification by the Florida Lottery that they are a winner. Failure to submit all required documentation will result in forfeiture of the prize.

U.S. Postal Service regulations include various restrictions on mailing Lottery tickets. The risk of mailing tickets remains with the player and will not be assumed by the Lottery.

ALL OTHER PRIZES

Winners of all prizes other than those listed above will receive their prizes, or a certificate describing the prize won and providing instructions for fulfillment, by mail within approximately fifteen (15) days from the date of the drawing.
